PrescribaRx’s MTM
(Medication Therapy Management) — Free to You
PrescribaRx's MTM program was developed to help ensure safe and proper medication use by Medicare Part D enrollees who have multiple chronic diseases, take multiple Part D drugs, and are likely to incur annual costs for covered Part D drugs that exceed a predetermined level. If you are taking multiple medicines for several chronic conditions, you may be eligible to receive MTM services. If you are eligible, you will receive a letter inviting you to participate, including instructions for enrollment.
If you choose to participate in our MTM program, services may include:- Medication therapy review by a MTM program pharmacist to ensure safe and proper use (e.g., review of prescription and over-the-counter medicines, possible side effects and drug interactions, and opportunities to help you reduce your prescription drug costs).
- Education to help you understand proper use of your medicines
- Access to counseling pharmacists.
- Involvement of multiple chronic diseases such as asthma, diabetes, dyslipidemia and hypertension;
- A drug regimen consisting of for at least ten covered Part D drugs; and
- Likeliness to incur annual costs of at least $4,000 for covered Part D drugs.
- Eligible beneficiaries will receive an invitational letter that explains the MTM services provided and provides enrollment instructions.
- Enrollment is voluntary. Members may enroll in the program by calling a toll-free enrollment number.
- An appointment with an MTM program pharmacist for an initial comprehensive consultation.
- This consultation may include a review of prescription and over-the-counter medicines, possible side effects and drug interactions, and opportunities to help you reduce your prescription drug costs.
- You, your physician, and/or pharmacist may also receive customized communications, which may include educational materials and/or drug therapy assessment and recommendations.
- You may also receive follow-up consultations to review your care plan and any medication changes.
