Special Care for Those with End-Stage Renal Disease

Through Universal American’s relationship with Fresenius Medical Care North America ("FMCNA"), the world's largest, integrated provider of dialysis products and services, we are able to help improve the quality of life and clinical outcomes for seniors and other people with Medicare with End-Stage Renal Disease through a Medicare Special Needs Plan exclusive to beneficiaries who require dialysis. The benefits and services of this plan are designed to meet the health challenges people on dialysis face daily.

Fresenius Medical Care Health Plan, a joint offering by Universal American and Fresenius, operates as a Medicare Advantage Private Fee-for-Service plan, although the plan contracts with nephrologists and FMCNA dialysis centers. Members enrolled in the plan have access to the Fresenius KidneyTel® program, a service designed exclusively for people on dialysis. KidneyTel provides for dialysis interaction with specialty nurse care managers and monitoring of members’ health status everyday at home including blood pressure monitoring. The plan provides nutritional supplements to address plan members who are challenged in maintaining proper protein levels. Additionally, plan members who are diabetic can receive diabetic testing and monitoring supplies at no cost to the member.

Currently, the program is offered in Connecticut, Massachusetts, New York, Rhode Island, Illinois, Alabama, Tennessee and California. Starting in 2010, the plans will be offered in the metro Ft. Wayne, IN area as Preferred Provider Organization. In our other service areas in 2011 the plan will convert from Private Fee-for-Service plans to Preferred Provider Organizations (PPOs).
